This screening identifies factors that may influence problem behaviors (PB). Use it as part of a comprehensive functional analysis, not as a standalone diagnostic tool.
Administer the instrument to multiple individuals who frequently interact with the client: parents, teachers, therapists, residential staff.
Answer each question by selecting Yes, No, or N/A. The charts in each section update automatically as you respond.
Upon completing all 16 questions, click “Generate Results” to obtain the complete behavioral functions profile.
Results should be verified through direct observation with ABC data and/or experimental functional analysis (EFA) before designing interventions.
